I got it too, once, for about a second, I don't remember what I did to get it though.

Yes. It is forty two afk... If i remember correcly, He was trying to experiment with his cfg files exec another cfg file or something, in a recursive loop....
edit: it may not be forty two, he probably was speccing me. I could tell from the last screenshot, look carefully and read.

I'm pretty sure It's the Forty-Two from AussieAssault. And by the looks of the last screenshot, it seems that he was in spectator.
It's probably fixed camera view, and when you jump, camera should too, so it's kinda bug :D
